MyHome Healthcare provides monthly visits focusing on patient wellness. During these visits, we will monitor vital signs, supervise medication management, and take the time to discuss patient thoughts and concerns. Visits can be provided in person, or via telehealth based on patient need and preference. In addition, MyHome Healthcare works with mobile x-ray and lab services to conduct diagnostic testing all within the comfort of home.
MyHome Healthcare believes that caring for our patients as individuals is paramount, and we always take the time to understand our patient’s values. Our patients deserve a clinical approach tailored to their unique health goals, and collaboration with additional support services is often necessary. Because managing these services can be challenging for those at home, MyHome Healthcare initiates services and collaborates directly with the agencies, always advocating for our patient to make sure their goals are met.
MyHome Healthcare supports relationships with all home health agencies to provide services (visiting nurses, physical therapists, occupational therapists, and speech therapists) needed to maintain the best quality of life at home.
MyHome Healthcare works closely with patient preferred hospice agencies, when the goal is comfort care and family support.

Far far away, behind the word mountains, far from the countries Vokalia and Consonantia, there live the blind texts. Separated they live in Bookmarksgrove right at the coast of the Semantics, a large language ocean.

There is always a provider on call 24/7 who can manage your needs remotely. If it is a medical emergency, we advise calling 911.
Yes. We will provide all of your primary care needs in the comfort of your own home. Most blood tests and x-rays can be provided by the mobile services we partner with.
For Medicare patients, Medicare will cover 80% of our visits and, if you have a secondary insurance, they should cover the 20% that is left. Most of our patients never see a bill from us.
For non-Medicare patients, you will be responsible for your usual co-pay after your insurance benefit has been applied.
It is no longer commonplace for primary care providers to follow their patients in the hospital as hospitals now assign a “hospitalist” or “intensivist” to care for you when you need acute care. This allows you to get the best care while in the hospital by a provider who is trained as a hospital specialist. When you go to the hospital we will provide an update on your condition and any relevant medical history to the hospitalist. When you return home from the hospital we will resume your care and review all hospital records and recommendations to ensure follow through of your discharge plan.
No, you can continue to see the specialists you have already established with. If we recommend that you see a specialist at any point in time, we will take care of the referral and will assist you with coordinating the visit.
I would say her strength about her work ethic is getting to know each patient and reassurance that they are in good hands. I would highly recommend Julianne as primary care.
Julie's ability to coordinate with the nursing staff and residents while providing the most personal and personable care possible is exceptional.
We are so happy to have Julie caring for my aging parents. She coordinates all of their care from the comfort of their home. We no longer have to juggle taking days off to shuttle them to appointments. Signing on with Julie has been a game changer for our family.
