MyHome Healthcare provides monthly visits focusing
on patient wellness. During these visits, we will monitor vital signs, supervise
medication management, and take the time to discuss patient thoughts and
concerns. Visits can be provided in person, or via telehealth based on patient
need and preference. In addition, MyHome Healthcare works with mobile x-ray
and lab services to conduct diagnostic testing all within the comfort of home.
MyHome Healthcare believes that caring
for our patients as individuals is paramount, and we always take the time to
understand our patient’s values. Our patients deserve a clinical approach tailored
to their unique health goals, and collaboration with additional support services is
often necessary. Because managing these services can be challenging for those
at home, MyHome Healthcare initiates services and collaborates directly with the
agencies, always advocating for our patient to make sure their goals are met.

therapists, and speech therapists) needed to maintain the best quality of

for home delivery.

that is reviewed and updated monthly, or more frequently based on patient need.
